I operate a fee-for-service practice, which means that I do not work directly with insurance companies at this time. However, if your insurance carrier is a PPO or a similar type of health insurance plan, you should review your policy to determine what type of coverage you have for "Out-of-Network" providers. If you determine that your health insurance plan covers "Out-of-Network" providers and you’re interested in using your health benefits, I would be happy to assist you to file your insurance claim. If you use your insurance in this fashion, you would be required to pay for services upfront. At the end of each month you will be provided with a superbill, which you would need to supply to your insurance carrier for reimbursement. Before commencing with treatment, I recommend that you contact your insurance provider to verify any benefits by asking the following questions:
What are my mental health benefits?
What percentage of each session is covered when I see an out of network psychologist?
Do I have a deductible? If so, what is it and have I met it yet?
Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
What is the amount of my co-payment?
Do I need authorization or pre-authorization in order to receive mental health treatment?

Health and Behavior Treatment
Couples therapy is only offered in 60-minute sessions. Please plan for both partners to be physically present during the session
If you do not show up for your scheduled therapy appointment, and you have not provided me with at least 24 hours advanced notice, you will be required to pay the full cost of the session. This is standard industry practice and is in effect since your time slot is reserved and cannot be offered to another.

Cash, credit/debit cards, and HSA are the only forms of payment accepted at this time.
